• If your child needs to take medication during school hours, the Medication Administration Consent Order Form must be completed prior to bringing the medication to the school nurse.
  
  

• Physical Exams are required in Grades 1, 6, and 11.
  
  

• Dental Exams are required in Grades 1, 3, and 7.
